The infection, D. You took antibiotics, to allow the swelling to go down. It is the confined spaces in the jaw that lead to the pain being so intense. Bone and tooth are not forgiving, in so far as the spaces within them. If the tissue swells, the nerve gets squeezed.

That is both from the infection and your uncorrected bruxism. There are many behaviours associated with bruxism, including holding the jaw clenched during the daytime. The muscles get sore, and the soft tissues of the gum get tender. Throw in an infection, and you're above the pain threshold. The infection-related inflammation may have physically subsided but the tissues are still healing. From the sounds of things, you really do need a guard while sleeping. Those teeth you have now are the best you'll ever have.
