Depends on the therapist, I think. Some write very detailed notes.

My therapist just rights down something innocuous because he's required to say something. Patient appears anxious. Or patient was distressed about work. Every once in a while he takes an actual note, usually about medication changes, or sleep changes, or something technical like that.

He knows that I, like many I suppose, really don't want our innards recorded and filed.
