Have you tried dopamine enhancing supplements? Like vitamin C, tyrosine, mucuna pruriens (contains l-dopa), green tea, stimulants?
I find the balance between serotonin and dopamine is important. All my drugs enhance both, synergistially. In the past straight SSRIs caused this really depressed feeling where I didn't want to go out or do anything. But you have to be careful because you can get a sort of false anxiety from being jazzed up with dopamine and norepinephrine. In that case exercise is CRUCIAL, as you probably know.
