My initial response is that you need to have the suggested answers listed before the questions. Sort of something like:
For Questions #1 and #2 answer:
no
yes, once
yes, occasionally
yes, frequently.
1. xxxxxxxxxxxx
2. xxxxxxxxxxx
For Questions #3 and #4, answer . . .
I'm sure you get the idea. I'll have to think about the questions themselves and get back to you.
