Inflammation a biomarker for choosing TCA vs SSRI?

Posted by SLS on January 14, 2023, at 15:48:28

There is sufficient evidence that Major Depressive Disorder is associated with inflammation.

These investigators found that high levels of blood C-reactive protein (CRP) predicted a better response to nortriptyline (TCA). Low levels of CRP indicated a better response to escitalopram (SSRI).

This type of investigation is clinical rather than being pure (neuroscience) research. The study was designed to be clinically applicable. Its objective was to test a new way to choose drugs for each patient without knowing the properties of the drugs being tested or the physiology of the brain.

My impression over the years has been that melancholic / endogenous depression responds better to tricyclics than to SSRIs.
