I asked my dr for that after coming on here and seeing a post about Ken Gillman. I consulted him via Skype and he recommended these 3 meds together.
50mg sertraline
50mg nortriptyline
56mg concerta (methylphenidate)
I've also seen evidence for methylphenidate as successful monotherapy in depression
