Anticholinergics and typical antipsychotics

(1) I've seen reports that retrospective studies have found an association between anticholinergic use and dementia: in the years preceding a dementia diagnosis, dementia patients have higher anticholinergic use than the general population.

Do you think that this is because (a) anticholinergics actually cause dementia, or (b) symptoms of the prodromal phase of dementia (e.g. depression, insomnia, anxiety) are sometimes treated with drugs that have anticholinergic effects or side-effects?

(2) I've seen reports that typical antipsychotics cause loss of brain volume. Is this because (a) they lead to actual neuronal death, or (b) due to a reduction in cerebrospinal fluid or reduced blood circulation?
