Hi Lamdage.

I switched from Abilify 10 mg/day to Rexulti 2.0 mg/day. I experience a more robust antidepressant effect now. Abilify left me somewhat flat. Rexulti has given me increased mental energy and motivation.

I'm sure you know that brexpiprazole (Rexulti) and aripiprazole (Abilify) are chemical cousins. They are very similar pharmacologically. However, brexpiprazole has a lower dopamine D3/D2 receptor binding ratio. The lower ratio should translate to a reduced risk of akathisia. My guess is that movement side effects are lower with both drugs when compared to most other antipsychotics. I base this guess on the fact that both drugs are DA receptor partial agonists (rather than full antagonists).

I am not convinced that brexpiprazole has a reduced risk of weight-gain relative to aripiprazole. Although my doctor has not seen weight gain with brexpiprazole, I am not losing any weight since switching between them. My appetite is not different. I am trying to change my eating habits a bit, so we'll see how that goes.
