also, do you have a GP, a family doctor who you trust, someone who you'd go to if you had a sore throat or bronchitis or whatever? You could ask that doctor for a referal to someone else about your ADHD, and that should be covered by your insurance (or your parents insurance)
You might just have to find a way to get your hads on your parents insurance card on the day of the appointment.
