If I have been informed correctly, the idea that combining buspirone (Buspar) and fluoxetine (Prozac) could lead to serotonin syndrome is based on the false or dubious assumption that 5HT1A (indirect) agonism is one of the causes of serotonin syndrome (SS). Buspar is a 5HT1A-agonist, and Prozac through its SRI is an indirect agonist of these receptors. I think newer research has shown that SS is not or probably not 5HT1A-mediated. So saying that this is a 'dangerous' combination is way overboard.
Any (S)SRI can cause SS *by itself*, i.e. *on its own*. So if pharmacies want to eliminate the risk of SS, they shouldn't sell any SRI anymore.
