My knowledge is limited but from a cv-neurological standpoint a trigger for migraine is that small paradoxical embolisms (air, small thrombus, vasoactive substances like serotonin) can go up to the brain through a PFO (something like a little hole in the heart) causing cerebral ischemia and following electrical depolarization waves, which means a migraine attack actually.
Maybe your fiance has a PFO that can be checked easily.
