When I took lithium, my pdoc told me that if I missed a dose that it would be best to wait until my next scheduled dosing time and just resume taking it as I normally would beginning at that time. In other words, if I were taking 600 mg twice a day (1200 mg/day total) and forgot to take my 600 mg morning dose, it would be best to first wait until the evening before taking my regularly scheduled 600-mg dose, and then to continue taking it as usual from that point forward (with my next dose being 600 mg the next morning).
What's most important is that you avoid "doubling up" or taking your doses only a few hours apart when you would otherwise be taking them several hours apart. That could cause your blood-lithium concentration to reach a toxic level.
It's possible that you might notice a brief reduction in lithium's effectiveness from skipping a dose. My guess is that the extent to which you might notice this may depend on your body/brain chemistry and the length of time for which you have been taking lithium, but any reduction in effectiveness probably won't be too drastic. But that's just a guess, not a professional opinion.
Of course, it would be best to consult with your doctor for direct instructions. I'm just letting you know what my pdoc told me to to when I was taking lithium.
