Effexor: timing of dose(s) in the 150-225 range

I've recently stepped up my dose of Effexor XR from 112.5 to 150 and now 187.5. (I had been taking 112.5 as a maintentance dose for two-and-a-half years but six weeks ago depression returned.) My pdoc had suggested taking the 150 in two doses, morning and night, but since my waking times have lately been erratic, I've been doing what I've always done, taking the full dose after lunch. What should I do with the 187.5 dose? The doctor suggested taking the 37.5 with breakfast and the 150 with lunch. (I should emphasize that he didn't say that this was imperative.) I had much rather take the full 187.5 after lunch (as usual).

I don't want to create spikes and troughs in my body's levels of Effexor. I thought the XR formulation meant that, even with higher doses, once-daily dosing was fine. What has been the experience of people here? Do people on 225 (a dose to which I may move up to in 10 days or so) divide their doses or take it all at once? Which is preferable (insofar as an objective answer is possible in this case), my pdoc's suggestion or my usual practice?
