Have you tried trazodone? It's relatively sedating but doesn't stimulate the appetite nearly as much as Remeron. Elavil (amitriptyline) does induce weight gain in some people, but it's not nearly as bad as Remeron (probably nothing is). The big problem with Elavil is its severe anticholinergic effects; in fact, they are so bad there is a school of thought that this drug's side effects are too severe to justify prescribing it. Amitriptyline is very widely used, however, mainly by GPs for sleep and pain--it might be a good choice for someone with fibromyalgia, for instance. It is also very, very cheap.
