I would stay on the Effexor and add the Lamictal to that!It sounds like Effexor may be cycling you, but most ADs will, and it's a far more "alert" AD than Zoloft! Which had me bumping into walls.
My pdoc added Lamictal. I think keeping what works, or what has high potential to work, or what is an optimum AD in terms of side effects. . and augmenting that, guided by comorbidities/ remaining symptoms. . is THE way to go.
I take Dex - Adderall is *wonderful* but a little too powerful for me! - and I still need a little Ativan to actually *go* to sleep. Again: augment. The pstim is, like you, a cornerstone for me, so a little Ativan is definitely worth it. Besides, it's my ADD chatter that never shuts up, at bedtime; a little benzo is the only thing that ever stops that.
Your Effexor dose is *way* low, BTW.
I can testify that the addition of Lamictal was The Single Thing, added to the rest of the package, that Vanquished All Depression.
My dx is ADD/BPII. . .but was mostly severe depression. . punctuated now and then by highs.
