Because I've always had depression and always felt this way, I'm not sure. Work especially. Nails on a chalkboard watching the clock.

Alchemy, the clock is rather oversized and hangs either above or beside the chalkboard. I might have seen them. I think of those as present/past.
For how much longer are your rTMS sessions going for?

Alchemy, it's the depression. 'You' are still there. And I'm assuming rTMS has begun. It might have you feeling odd.

I had my 7th treatment of rTMS this morning. I haven't noticed anything and am feeling discouraged. But there still is time to respond somewhat. I have 30 treatments.
But now I have to think about what if it doesn't. Meds make me worse. ECT or the d word. I've been worse, but after 25 years of depression it's mostly the hopelessness. From my experience, I concluded that ect only helped me when drug trials really screwed me up. It was like a "reboot" from a hung-up application. But it didn't take away my normal "state". The dr. said that he thinks that if I do it, it would be a more intense treatment. Probably bilateral, more treatments, and more maintenance. God let TMS help me!!!

The doctor I spoke with said improvement can be slow but steady. Some maintenance med is required in the majority of cases but not all. And that people go for maintenance rTMS every so often.
How often per week are you going?
Do you have a therapeutic alliance in place for when the sessions are over?

I was talking about more intense ECT treatment. But I don't know how much I am up to it or have hope for it. I came home early from work, but I'm doing that too much. I've thought about short-term disability, but I'm too messed up even when I'm not working. I've never been this serious before about not hanging in there - if rTMS doesn't work.

5 days a week - today was my 8th. There is still some hope, but it's not looking like I'm on the improvement path.

Alchemy, when you say it looks like you're not on the improvement path, may I ask what this is based on? Is your doctor saying things? Also are there response indicators that are missing according to you and according to your doctor?
I'd like to know, so you aren't giving in too early. There are probably reasons that rTMS is done in 30 sessions. Some people may not show indications of response for some time. I don't know.
Write back when you can. Let me know what your doc says if you are comfortable sharing that.

The intake coordinator does my sessions now that the dr. has evaluated the settings. She is give me more hope that the dr. probably because he doesn't want to give false hop. She said that many start to notice arond 7-10 sessions. I asked her today about other people talking longer - she said yes. And that there was one man that they had almost given up on that felt better on the 20th.Some of my discouragement is that it seems like in the last couple days I might be feeling worse. Worse is not good.

I'm going to think of you feeling worse as a blip for now. I think it's so difficult to not be hypervigilant after repeated treatment failures and given that your stakes are high.

I'm sorry you feel worse no that's not good. So will you do the ECT do you feel? Finish out the rTMS? Phillipa

Just a thought: Did you get possible underlying physical problems checked?
Testosterone/hormone levels (endo doc), heavy metal exposure anywhere? Food intolerances/allergies? Large bloodwork.. Candida
There are lots of things that could contribute a great deal to your condition
