Some therapists do work with this. I remember once reading about whole social skills groups, which I thought was a wonderful idea though unfortunately there are none in my area.
One thing I've found helps me is to scan the group for someone who looks as uncomfortable as I feel, then go up and say hello. I always feel way more comfortable than trying to engage with people who look very comfortable talking to each other.
I'm really bad at social rhythm, and find large social gatherings to be a bewildering and overwhelming experience. But groups that are purpose driven, like clubs organized around a hobby or charitable purpose, give a built in topic of conversation.
