I think the starting point to have a discussion regarding depression is to first define what depression is. I have experienced two very different things that have both been described using but one word - depression. One experience is bipolar depression in the absence of psychological depression. The other is psychological depression in the absence of bipolar depression. They feel very different, and they respond very differently to treatment. I once thought to describe major depressive disorder and bipolar depression as biological illnesses with symptoms that mimic depression.

I would like to see a study of the effectiveness of psychotherapy in people who have been diagnosed as having MDD and who are DST non-suppressors (dexamethasone suppressor test). I bet psychotherapy is found to be ineffective in this subgroup.
