Speaking as a former manager, your excellent track record will speak louder than anything you could do now. Unless the world stops if one person is absent (and if it does, they need to reorganize the way they have things set up) they would rather have an excellent worker most of the time, than a poor worker all of the time.

I had one woman working for me who got incredible migraines every month. She had to go home. She was usually gone 4 - 6 hours and then came back to work. She told me about the migraines and how she would like to handle them. If she was working on something time critical she would show me or someone else where she was. She was always apologetic about leaving (but she didn't go on and on about it). I was very happy to have her in my group.

Good companies meet their employee's needs.
