Get a comprehensive blood test done, that's the first thing to get out of the way. thyroid, H. Pylori (bacteria that leads to peptic ulcer), and vitamin deficiencies (like iron, B's, etc.) should all be checked out, and any irregularities be smoothed out. Then after you re-test after any problems were found and corrected, you and your doctor can feel less vague about zoning in on what's causing all of this.

Maybe even a whole new physical, from your current GP or new one. Make sure to mention it not just to your p-doc-- p-docs may sometimes get so use to assuming the chemical origins to problems that they sometimes forget the basics might cause glitches to occur, like heart problems, thyroid, etc... so don't rely on your p-doc to answer your questions, this is something that your GP should comprehensively examine, then your p-doc review it, and if by that point your p-doc can't resolve it, he'll refer you to a neurologist.

this is just standard stuff. It'll be reassuring for you to know that your GP has tested all he can with your blood and urine, that your vitamins and hormones are okay, or if not, you know exactly what to resolve to make them better.

Just remember that resolving an irregualarity, while critical to your health, may not resolve your problem, because they may not be related to your troubles.

In the mean time, while you're waiting for blood tests and the like, have you tried a stimulant like Adderall or Provigil or Dexedrine to keep you normalized during your lulls? Have you considered re-adjusting your sleep time? Maybe even going to sleep earlier even if it's not natural for you to do so (through sleeping pills) or waking up later (if your situation allows it)?

I'm more concerned over your shaking than your fatigue, although I'm not a doctor and don't have a clue. Have you tried a B-100 supplement? Or at least testing your B counts? The B's provide energy (not execessive, just normal energy in cases of atypical fatigue) and help heal damaged nerve cells. Shaking... sounds like nerves are frayed, possibly. Again, I'm making this up, I'm not a doctor.

Anyway, I take a B-100 complex (I'm vegan), Iron pill (I'm vegan), Lecithin (for nervous system, although I've never read any research on it), Vitamin E (for acne, although this is not clinically established) and Vitamin C as needed for mild sickness.

I'm a morning person- feel great for first couple of hours, then come crashing down at about 11 or 1 or so, enter a few hours of lull time, come up again... but I'm going to try taking Adderall during my lull to offset this, see what happens.

But seriously, what time do you go to sleep? Let's say you wake up at 8AM. Think about trying to go to sleep at 9PM for a solid week with the help of a sleeping pill (Benadryl or Ambien or Sonata). This shouldn't be unorthodox in your case, since it's possible inducing heavy amounts of sleep may resolve your problems... although it seems more complicated than this, it's worth a shot?
