Hi-
Sending a handwritten letter, should be as effective, however, they can always 'claim' they never received it. I sent everything certified.
> Hi, I want to do the same thing, I sent a letter, hand written, just saying please send me a copy of my records, it has been about two and a half weeks. I want to have a record of all the meds I have tried. But I have not heard anything yet.
>Perhaps you could call to verify they received the letter? Ask if there are any problems, etc... if they say they haven't received it, send it certified... much harder to dispute.
> You say 'send a form in writing' is there a particular kind of form to recind the med release?
> I was thinking if I didn't hear anything in another week or so, I would write again with a certified return receipt. This post reminded me.
> Thanks.
>I can't think of a particular form... however, I'm not an expert(not legally..hehe, just dealt with this for what seems an eternity) You can search on the web for a template, or if you'd like to exchange email addresses, I can send you the one I have. Anything I can do to help out.
