Kohut has some interesting stuff on narcissistic personality. He thinks it results from lack of empathetic attunement. Basically, when we didn't receive the nurturing, empathy, and care that we needed as infants. Many therapists consider narcissistic personality / borderline personality / hysterical or hysteriod personality to be very similar. Theorists differ with respect to how much they focus on ego and counter-transference reactions rather than empathetic attunement and environmental experiences.
