Well, I was going to leave it out, but those are the standard FDA categories. See, for example, the Medwatch site:

The FDA is concerned with drugs, of course. But (1) the form here is for any adverse events that occur during participation in PB and not only those that are caused by it and (2) if a congenital anomaly were caused by a drug that someone took because of information here, PB could be considered to have played a role...
