Luvox's Effect On the 3A4 System For Metabolism

Posted by bulldog2 on January 31, 2008, at 14:56:24

I recently had my thyroid tested and found all my values had increased quite a bit without increasing my synthroid. I was baffled until I found this writeup by a Dr. Jones. Of course i'm now wondering what other hormones Luvox might effect. Is it increasing testosterone, estradiol etc.?

See Dr. Jones comment below on luvox

Answer: I wouldnt say Im down on Luvox. I do have a few patients who seem to do well on it. The reason I try other serotonin reuptake inhibitors is that Luvox has the most drug-drug interactions in that group of meds (Prozac, Zoloft, Paxil, Celexa, Lexapro, low dose Effexor XR 37.5-75). For example, Luvox blocks the metabolism of caffeine which may be the reason it causes more insomnia. It can cause more daytime sedation. It also blocks the metabolism of all the meds (and hormones) in the 3A4 category (the biggest group), so that can make things very complicated.

Not sure...that's what I want to know...But I know you take thyroid and just be aware that as you adjust your luvox up and down (if you do that) your thyroid levels may vary considerably.

My dearest Phillip...If you read my original post I mention my thyroid values increased dramatically after I started luvox 50 mg without changing my dose of synthroid...It made no sense until I read the dr. jones article...

(Note: Not all drug interactions are known or reported in the literature, and new drug interactions are continually being reported. This information is provided only for your education and for you to discuss with your personal healthcare provider. )

LEVOTHYROXINE (in Synthroid) may interact with GRAPEFRUIT JUICE

Levothyroxine is absorbed in the intestine. Grapefruit juice may slow this process down causing levothyroxine uptake into the body to be reduced. Available evidence suggests this is a small interference, which is not likely to be important. There are no special precautions with taking levothyroxine and grapefruit juice at the same time. You may want to ask your healthcare provider about this potential interaction if you think you are having problems.

This interaction is poorly documented and is considered minor in severity.

ALCOHOL may interact with DIAZEPAM (in Valium)

Although the cause of this potential interaction is not fully understood, intolerable or otherwise undesirable side effects may occur when diazepam is taken with alcoholic beverages or alcohol-containing products (such as some cough syrups and elixirs). Potential side effects include drowsiness, dizziness, blurred vision, headache, and nervousness. Because the combination of diazepam and alcohol (ethanol) may impair your judgment and/or reflexes, it may be dangerous to drive or to perform tasks which require you to be alert after taking these together. It would be advisable to avoid consuming alcohol while you are taking diazepam. Discuss this potential interaction with your healthcare provider at your next appointment, or sooner if you think you are having problems.

This interaction is well-documented and is considered moderate in severity.

FLUVOXAMINE MALEATE (in Luvox) may interact with DIAZEPAM (in Valium)

Fluvoxamine may block the breakdown of diazepam by the liver. If this happens, blood levels of diazepam could be increased and this could cause an increase in side effects. Side effects that may occur with the use of diazepam include prolonged sedation, dizziness, confusion, and breathing difficulties. Use caution when driving or performing tasks that require careful attention. If fluvoxamine and diazepam are used together, your doctor may want to monitor you for side effects. If you are experiencing problems, it may be necessary to adjust the dose of diazepam. Your doctor may also consider using an alternative for diazepam. You may want to ask your healthcare provider about this potential interaction if you think you are having problems.

This interaction is poorly documented and is considered minor in severity.

Potential side effects include drowsiness, dizziness, blurred vision, headache, and nervousness. If possible, avoid taking diazepam with grapefruit juice.
