Have you tried an AAP like Seroquel to calm the anxiety and agitation? Like a low dose? I'm thinking with something to clamp down the anxiety and hypomania type symptoms, you could be more aggressive about using energizing meds.
Have you tried smoking/vaporizing/eating marijuana for pain relief and antidepressive effects? That one can go either way, but worth a try. I read an interesting article about how a mother gives her son pot cookies for his autism. Not the same thing, but marijuana doesn't have to be seen as some crazy therapy or without therapeutic effects.
Chronic use is fraught with difficulties as well, but we're just trying to get you out of bed and functional, right?

I did try MM ingested. It wasn't successful for me, and I was trying to avoid all the stuff thrown at me--cymbalta, lyrica.
(I couldn't tolerate seroquel, either.)
I'm here on this board trying to reach back into some of my old TCM stuff and learn about herbal support. While I think about my next possible med ramp-up :-/
