Does AAP dosage usually go up or down in psychotic and severe mood disorders? I'm just curious. I usually take 30 Abilify, so I'm maxed out. Sometimes, I'll take 2mgs/Risperidone w/ some gabapentin at night.
I don't know what I'd do if I needed more AAP action on a regular basis. Seroquel?

there's geodon......it's not really potent in some of the sedation, but they can use it with other AP's i was on a super maxed out dose of 160mg and zyprexa 10mg, the doctor through my hyperactity was from mania, it totally knocked me into still wonderland....i just walk around didnt feel the usal things i felt.....
but geodon is used as matience for mood and psychosis disorders
zyprexa is strong and sedating, and makes you not euphoricaly doped
you could ask about invega....

I am guessing that Abilify has greater utility treating the negative symptoms of schizophrenia than positive symptoms. You can add a second AP for positive symptoms or as a hypnotic. Risperdal is usually better for negative symptoms, but not for sleep induction. Seroquel should help with sleep, but may not be as effective for positive symptoms. Actually, I see Abilify interfering with Seroquel in a way that necessitates dosage escalation of Seroquel to 800 mg/day or higher. Interestingly, some people report that Abilify helps with anxiety, but I am seeing anxiety emerge with its discontinuation in people who had no anxiety before its introduction.
If sleep and/or anxiety are the primary symptoms you need to address, I think Seroquel makes sense to add. If paranoia and depression are involved, Saphris is worth exploring. Of course, in the absence of positive symptomatology, your choices increase. Perhaps low-dose Remeron taken at night makes sense.
I hope other people chime in on this.

Both, undoubtedly. Apologies if that was a bit trite. Don't know if that's the right word. It's true though.

strange no one mentions zyprexa, aside of metabolic problems it's pretty damn good for anything

You know what?
I kinda like Zyprexa when i take it as an emergency med.
It really calms you down. Its like putting a comforter around your shoulders that protects you.
Its like "i dont want to feel anything anymore". Thats when i take my emergency Zyprexa.
Only if im pre-psychotic or full blown psychotic of course.

I'm just wondering...over the long haul, could I maybe do a dosage reduction? I hear you hit 30, 35 w/ Abilify and...that's it. End of story, lol.
The 2mgs/Risperdal+200-300/Neurontin at night really helps. I just don't do it every night, because I'm weird about 2 AAPs long term. I dunno.
Seroquel is too sedating. Zyprexa scares me because of metabolic issues...HOWEVER, I was thinking about asking about metformin+zyprexa (dissolving...not only do I like the idea of maybe less weight gain, I figure it'd be like anti-crazy candy). That'd be more sedating, which wouldn't be terrible...
...but I do like Abilify. Non-sedating, no EPS, etc. I think tolerability is the big selling point for Abilify.

Shrinks tend to shy away from prescribing to many antipsychotics because of the risk of EPS, including tardive diskenesia.
If you're already taking 2 mgs of Risperidone at night, your best bet would be to take that on top of the Abilify.

"Risperdal is usually better for negative symptoms, but not for sleep induction."
I meant to say that that Risperdal was better for positive symptoms when compared to Abilify.
