EMSAM - Day #1 (6/13/06)

DX: BPII, w. Severe Generalized Social Phobia

MEDS: 2mg./day Clonazepam, 400mg./day Gabapentin, 25mg./day Topamax (+other non-psych.)


Put 1/2 of 20mg. (6mg. per 24hr.) patch on at 11am.
@ 1 hr. felt start of increased energy.
@ 2 hrs. mood improved.
@ 3 hrs. mood improving greatly, maybe start of hypomania.
@ 4 hrs., still very good mood/hypomania, but increasing anger/irritability/hostility/agitation/self-hatred.
@ 7 hrs. removed the patch because I could no longer stand the increased hostility/irritability et. al.

De-stabilized for days after. Afraid to re-try 'til could get to speak with my doctor.

If EMSAM is supposed to alleviate anxiety, I'm not seeing it on this board. My anxiety overrides any depression although they go hand in hand. I have not used my EMSAM yet and I am printing out some of these posts and taking them to my psychiatrist today. She cannot force me to take anything. I would rather just stick to my xanax, fairly low dose. Val

I suspect your "speed" reference is pretty accurate. See this excerpt from EMSAM's PDR entry:

"In vivo Metabolism

Transdermally absorbed selegiline (via EMSAM) is not metabolized in human skin and does not undergo extensive first-pass metabolism. Selegiline is extensively metabolized by several CYP450dependent enzyme systems (see In vitro Metabolism). Selegiline is metabolized initially via N-dealkylation or N-depropargylation to form N-desmethylselegiline or R-(-) methamphetamine, respectively. Both of these metabolites can be further metabolized to R-(-)amphetamine. These metabolites are all levorotatory (l-) enantiomers and no racemic biotransformation to the dextrorotatory form (i.e., S(+)-amphetamine or S(+)-methamphetamine) occurs. R(-)-methamphetamine and R(-)amphetamine are mainly excreted unchanged in urine. ".

It has helped my anxiety alot. Maybe I am the only one on the board who has been helped or am the only one to be mentioning it??

Donna,

I haven't posted on EMSAM in a long while, but FYI, its continuing to work well (day 58). You just don't have the inclination to post about it on the NET (at least I don't) when something is actually working. As far as it being like "speed", I have no idea what speed feels like, haven't never desired to, let alone tried it, but EMSAM in no way feels like a stimulant personally. I just wanted to post my .02 having seen a number of unsatisfactory results for other pbob posters.

Thanks for chiming in aabag, I was beginning to feel like the sole advocate. Maybe it is particularly helpful to people such as myself who when depressed sleep all the time or lay around without an ounce of energy to do anything, in spite of the accompaning anxiety. If one is already restless and sleepless, I can see how EMSAM may aggrevate this. But for me, it is just what I needed.
