Imipramine versus desipramine

Posted by Squiggles on August 16, 2005, at 14:20:18

I think i did not press the right
button to send my message previously;

To any phychopharmacologists here,

What is the Desimpramine equivalent of 150mg Imipramine? Suppose a person has been taken
of Imip. 150mg and put on 200mg Desip. how
long can one reasonably expect depression
to lift?

Thank you

The two drugs are approximately equipotent with respect to dosage.

Imipramine gets more people well than desipramine. It is probably logical to first find the dosage of imipramine that works before switching over to an equivilant dosage of desipramine. If you lose the antidepressant effect once you crossover to desipramine, you will know that the additional properties of imipramine were necessary for the therapeutic response.

I forgot to mention that desipramine is the immediate metabolite of imipramine. If you take imipramine, you will be supporting a therapeutic blood level of desipramine at the same time. If imipramine doesn't work at high dosages, it is unlikely that desipramine will.

If side effects with imipramine are intolerable despite an antidepressant response, but desipramine proves inadequate, you can try either adding some Prozac to the desipramine or switching to nortriptyline.

Thank you Scott,

You are one psychopharmaceutical geek :-)

Well, if i had done it, i would have added
desip. to the majority dose of imip. but
hey, i'm just a dr-wanna-be;

Thanks again.

Imho, i think desip.(200mg) is too weak compared
to imip. at 150mg - it's only the 2nd day, but
my experience shows that a drug slams you
at first and then feels weaker as you adjust.

I am quite worried about this person - there
has been a 60 lb. weight loss in the past
8 months or so - i am a bit surprised that
the doc. puts that to depression - to me it
looks like a general exam is necessary, esp.
after a gastro operation and the use of
Serzone with a history of Hep.

But then again, i do worry in general about
everything, so i guess the dr. knows best.

Third day on desipramine. It's starting
to get better. I apologize for the compulsive
posting. It has been such a long trip
in misery and drug testing, that I have
become a bit upset, and with the recent
35% body weight loss, after a gall bladder
operation, i do worry that there is another
problem. For example maybe the imipramine
doesn't work because the liver has been
effected by Serzone, blah blah blah, speculate
speculate speculate.

I am pleased that the 3rd day is better than
the 2nd on desip.

I would have liked to try imipramine. My doctor wants me back on SSRI's though.

Too bad.

Thank you Scott, and others who have helped
with the imipramine/desipramine question.
From what i read on the net desipramine *is*
weaker at least in side effects, and does
seem to take a larger dose than imipramine to
work. At 200mg it was just weaker than
imipramine at the same dose, and i suggested
to my friend to raise it; and her dr. ok'd
a dose up to 300mg - less side effects, equivalent
antidepressant effect.

Thank you.

Yes!
