TCA with least sexual s/e?

Hi,

some of you maybe have followed my threads. And those of you know that i can't stand the current treatment with 45mg remeron and 100mg zoloft. Ed and i have discussed that aurorix could be an option and that i will bring it up with my pdoc at next appointment. However, if the idea is rejected by my pdoc for some reason, i may have to go back to the old TCA's and i want something to propose. I've tried clomipramine in the past and it worked for depression but i couldn't stand the side effects. Nausea, constipation but worst of all, the sexual ones. Not able to reach orgasm.
What could be an alternative to clomipramine? A TCA.. I can take nausea, dizziness and so on in some amount.

Wellbutrin (bupropion) is often used with SSRIs to counteract their sexual side effects. If you need to go off Remeron, then Wellbutrin might be a good substitute. It increases NE as Remeron does, but also increases DA, which may account for its pro-sexual side effects.

Hi Tensor,

Are you in Europe?, then there are plenty of options. From my experience I can number some of the TCAs without sexual side effects.

Amitriptyline (Tryptizol in Europe. Elavil in US)
Dosulepin (Prothiaden in Europe. Not in U.S)
Lofepramine (Gamanil in Europe)

Or you can try Maprotiline (Ludiomil). It is not reported to have any sexual side effects.
I personally prefer Amit or Dosulepin. AMT is more effective than prothiaden.

Hi Mattias,

There's nortriptyline to consider. The brand name in Finland is Noritren. I think lofepramine is available in Sweden, not sure about Finland. In Sweden the brand name is Tymelyt.

I think nortriptyline has the lowest sexual side effects of the tricyclics, based on what I've read and from my own experience. Lofepramine, which is a prodrug (precursor) for desipramine, is also considered to be good, but my own experience is that desipramine lowered my libido. However, in general, noradrenergic drugs like this to do not tend to cause orgasm difficulties as the serotonergic drugs like the SSRIs and Nardil do.
