MAOs and Ultram together?

Posted by ZenTen on November 29, 2004, at 11:01:35

Hi, I've recently been asking questions on here regarding Nardil/Parnate,as my psych is going to most likely be putting me on it in a week. Currently, I am only taking Ultram/tramadol at 50mgs TID, for pain and Ambien as well as Seroquel 25 mgs or Zyprexa 2.5 for sleep. I've been on this med for a long time. Incidentally, Ultram also helps quite a bit with my depression on it's own! More so than any AD with no side-effects! Though its effects have waned after 7 months of use.

I know that Demerol is seriously contraindicated with any MAO and other opiods seem ok in half doses. Ultram, as far as I am aware has slight opiod properties, as well as low serotonin and norephinpherine binding and is somewhat similar to Effexor chemically. This in itself seems to makes it a big no no, to take with Nardil or Parnate.

I havent seen any literature on taking the 2 together.

Does anyone know anything about this or does anyone take Ultram/tramdol and an MAO together, or have any experience with it?

I was surprised to find that neither drug monograph for Nardil or Parnate specifically lists a warning about combining with tramadol. I thought the serotinergic activity of the latter would be enough to increase the risk of serotonin syndrome into the "concerned" zone, if not "risky".

I think it would be prudent to understand the symptoms of serotonin syndrome, and have an idea how to manage it if it does occur.

I knew I'd seen it somewhere. It's in the tramadol monograph. To quote, under the heading WARNING: "Use with MAO Inhibitors: Tramadol should be used with great caution in patients taking monoamine oxidase inhibitors, because animal studies have shown increased deaths with combined administration."

Those deaths are caused by increased intercranial pressure and seizures.

Do you have any luck with Vicoprofen? I took that while I was on Nardil after some dental work. I did not experience any drug interactions, and the pain med was effective in cutting the pain from my gum surgery.

Public Service Announcement: Floss everyone! It's worth it to avoid gum surgery!

Actually, I take the Ultram more for mood effects that for pain--I get it from my GP. It has drastic mood elevation properties for me... More so than vicodin and oxycodone for some reason. It really doenst help my back pain and migraines to any appreciable extent. It is the only thing that has some effect on my depression and anxiety. I am losing its effects though. I guess Its pooping out.

I think it helps my depression some because I must be a slow metabolizer and am probally accumlating it's metabolite M1,ithits the opiod mu receptors. That's all I can figure.

My Psych thinksI have opiod responsivedepression ashe calls it and has recommeded bupherone (sp) however, she hereself wont Rx me an opiod. So it does me no good at all because I cant get it.
I know it sounds cliche and people say it a lot when they say, "opiods make me normal" not high. Well I dont usually buy that but I can say that since Ultram isnt a classical Opiod and is supposed to have less abuse and technically no abuse potentional and I respond to it better than stronger ones, that truley there is something else going on.

I was hoping to augment it with Nardil but this doesnt seem to be a really good idea. And in fact appears deadly! I am a little upset because I wanted to try Nardil to treat my house-bound social phobia and a set of other mental issues that keep me from having even a glimce at a ''normal'' life.
