Which of the SSRI's are the most potent?

Posted by BRC on May 5, 2004, at 16:01:43

Hi, I asked this question earlier to that "WIZ" (complement) Chemist. But I'll ask this question to anyone who will lend me their knowledge.
Out of all the SSRI's which one's exhibit the most powerful antidepressant effects? Or do the drugs vary from person to person? Or are they all about the same? I take Lexapro and was wondering how it stacked up against other SSRI's. Thanks in advance for anyone who responds to this post.

I think that in practce, its hard to answer that question since it greatly depends on the dosage.

Hi,

Statistically, Zoloft is the SSRI that has proven to be above average in terms of efficacy, but if Lexapro agrees with you, I would stay on that as it appears to be gathering a reputation as a likeable & effective AD. The biggest difference between SSRI's is their side effects, more so than their efficacy, so most people tend to have a favourite.

Cheers,
Panda.

Paxil is known to be the most potent, which is why the horrible sexual side effects. This is not a guess this is based off studies.

Is that paxil is more potent mg for mg ?? or does that mean paxil is more potent at it's theraputic dose ??

I believe that Paxil has the highest affinity for the 5-HT transporter, while Celexa and Lexapro have been deemed the most selective. I don't know, but maybe Lexapro is more selective than Celexa.

Potency when used in this context refers to pharmacological properties, and not necessarily clinical efficacy. As Panda points out, Zoloft is often considered to be the more effective antidepressant, yet it is the least potent at binding to the 5-HT transporter.

Thanks, for everyone's response to my post. I asked the question which SSRI is the most potent because I have been on Lexapro for over a year and my depression has not lifted. I don't know whether to ask my pdoc to up the dose (I currently take 20mg) or whether to try and start all over againg with a new med. I have tried other AD besides SSRI's such as Effexor working my way up to the max. dose, but had to many side effects (trembling, muscle spasms etc.)

I have tried most all of the SSRI's, but really wasn't on them long enough to reach their full potential. So, I am in a delima. Do I start searching for a new med., up the dose of my current, etc. Anyone's input would be greatfully appreciated. Thanks!!

Has it improved at all? If there has been a slight improvement, then go for a dose increase. There really is no upper limit, except that dictated by side effects.

> I don't know whether to ask my pdoc to up the dose (I currently take 20mg)

How long have you been taking 20 mg per day? For a year? If you've been on 20 mg per day for at least eight weeks, with slight improvement, I'd bump it up to 40 mg per day.

Thanks Keith for the advice. I have been on the Lexapro for a year for depression and ocd. I can say it has helped some and will probably ask my pdoc for an increase. But I didn't know that Lexapro could be increased beyond 20mg? I thought that 20 was the max. dose. I see my pdoc. Tuesday and will ask him to see if he will increase it, but I don't know if he will or not?

Thanks for your response. Everyone's support on this board is greatfully appreciated.
