What dosages of Remeron are usually necessary for severe depression?
I have been at 60mg for about a week - added to imipramine and Effexor. Do some people need 75mg or more? I've heard that in Europe, some doctors are pushing Remeron up to 120mg!
Thanks.

The recommended dosage is 15mg 30mg or 45mg if you look at "http://www.rxlist.com"
I am taking 30mg and started a Babble thread called Remeron experiences.

>Scott...after a consultation with yet *another* psychopharm yesterday, he said that upper doses as you mentioned are really only limited by side-effects. He mentioned to me about Prozac, going up to 100mg's being no problem as one example. I don't know about toxicology of Remeron, so you may want to speak to a pharmacist, but if no problems, I don't see why not push the dose to get best effect.
Best wishes,
Jay

Hi Scott,
I felt like doing a little research this morning-hope this is helpful.
TammyI think the post below answers your question in part:

www.dr-bob.org/babble/20020525/msgs/107668.html
From the above:
There's not too many of us maxiRems, as far as I can tell. Did you know that in Europe they regularly start them out at 30 mg and the typical dose is 90mg? I'm very glad I had the intuitive hit to increase my dosage from 45 to 75mg. It is an entirely different drug than the dopey half baked response I was getting at lower dosages. I think Remeron is wonderful and wish the US psych community would get a clue as to it's effective dosages.http://neuro-www.mgh.harvard.edu/forum_2/GeneralNeurologyF/1.2.0011.11AMREMERONDOSAG.html
From the above:
It's important to note that Remeron is unusual in
that there are more unwanted side-effects at lower
doses, esp. sleepiness. The neurologist who did
the original Remeron study, Dr. Virginia Pact,
strongly recommends a STARTING dose of 30 mg.

From the above:
DOSAGE AND ADMINISTRATION
Initial Treatment
The recommended starting dose for REMERON(r) (mirtazapine) Tablets is 15 mg/day, administered in a single dose, preferably in the evening prior to sleep. In the controlled clinical trials establishing the antidepressant efficacy of REMERON(r), the effective dose range was generally 15-45 mg/day. While the relationship between dose and antidepressant response for REMERON(r) has not been adequately explored, patients not responding to the initial 15 mg dose may benefit from dose increases up to a maximum of 45 mg/day. REMERON(r) has an elimination half-life of approximately 20-40 hours; therefore, dose changes should not be made at intervals of less than one to two weeks in order to allow sufficient time for evaluation of the therapeutic response to a given dose.
