I want to take a combo of the MAOI Marplan + Nortriptyline + St. John's Wort + SAM-e. Am I going to die? Do I need to carry oral nifedipine with me just in case? Or the anti-serotonergic cyproheptadine? Help!

well...some docs have combined MAOIs with low doses of a TCA. I would say that adding SJW and SAM-E to this combo would be dangerous...You may not want to combine cyproheptadine with any of these meds, it exacerbates the anticholinergic effects...
The MAOI would be a potent enough antidepressant for most people.

The nortriptyline should be fine. I'd ditch the SJW and the SAMe -- they do have serious potential to inderact with Marplan.
How long have you been taking each of these?
Nifedipine, incidentally, is for hypertensive episodes. It's not usually something to use for the "serotonin syndrome," for which there is no specific treatment. I think it's a good idea for anyone on MAOIs to carry nifedipine, though.

Hey dude don't try this combination, life is too precious to waste. If you do this combo have someone there so they can call 911 for you. I'm not being negative, just trying to save your life. Try Nardil, Valium and Neurontin the most potent combination I know of. It will have to help. Make sure you take enough of the Nardil, too many doctors error on the stupid side of caution. The formula for Nardil is 2.2 mg. per lb. of body weight. If in between go higher! Hope I've been of some help.
