There's a lot of debate about this combination (mostly debate over SSRI + Trazodone); what is the conclusion of that debate--is this combo safe?

Not aware of any debate, but took trazadone off and on for some years for sleep, with Effexor. Course I tend not to lump Effexor in with SSRIs, which were all uniformly ghastly for me.
